Burton Blatt Institute It is with great pleasure that the Syracuse University Burton Blatt Institute endorse the New York State Institute for Health Transition Training (NYS IHTT) to promote self-determination, optimize health and well-being, and improve access to health care and the quality of health care that is delivered to young people with developmental disabilities in New York State. The mission of the Burton Blatt Institute is to advance the civic, economic and social inclusion of people with disabilities in a global society. Our participation as a founding partner of the NYS IHTT helps fulfill this mission and increased our capacity to improve the lives of New York State youth with disabilities.
Cerebral Palsy Associations of NYS Cerebral Palsy Associations of NYS is pleased to endorse the New York State Institute for Health Transition Training. Our mission at CP of NYS is to advocate and provide direct services with and for people with cerebral palsy and other significant disabilities, and their families, throughout New York State, in order to promote lifelong opportunities and choices for independence, inclusion, and enhanced quality of life. Our organization’s mission and goals are in keeping with the efforts of the NYS IHTT as it works to promote self-determination and improve access to quality health care for young people with developmental disabilities.
Exceptional Family Resources Exceptional Family Resources (EFR) is happy to endorse the NYS Institute for Health Transition Training (NYS IHTT) in it’s mission to promote self-determination and improve access to quality health care to young adults with developmental disabilities in New York State. Parent Network of Western NY Parent Network of Western New York proudly endorses the NYS Instittue for Health Transition Training and their mission to promote self-determination and improve access to quality health care to young adults with developmental disabilities. Parent Network offers the tools necessary to empower families to successfully advocate for their children and young adults with special needs. The partnership between Parent Network and NYS IHTT will allow us to further our mission of helping families and professionals enable people with disabilities to reach their own potential.
Parent to Parent of NYS When parents have children with disabilities they become caught up with handling all of the items that need their attention in the present moment. We are often unable to do all of the things everyday that need to be done…work, homework, child care, medical care, physical therapy, specialist appointments all consume our time. On top of this balancing act, we try to be parents instead of case managers, social workers and care providers.Before we know it, we are in the final years of school. Where did the time go? What’s next? How do I help my teenager move on and move out? Who will his physician be when he can no longer access medical care through his pediatrician? What support will he need to sustain an independent lifestyle? HealthyTransitionsNY.org is a place that has resources for youth and parents. The Healthy Skills Checklist outlines many of the topics that need to be considered as a young person moves into adulthood. The Learn, See, Do approach offers opportunties for youth to build on their health care self-advocacy skills. This website is a valuable tool and resource for understanding terms and developing health care advocacy skills.

Syracuse University Center for Digital Literacy The Center for Digital Literacy at Syracuse University is pleased to endorose the New York State Institute for Health Transition Training at SUNY Upstate Medical University in it’s effort to provide quality curriculum and tools that benefit young people with disabilities. CDL and the Institute share a commitment to interdisciplinary and collaborative research and development focused on the impact of technology on society.
Syracuse University School of Information Studies The iSchool at Syracuse University is pleased to provide a strong endorsement of the New York State Institute for Health Transition Training.
