Parent to Parent of NYS When parents have children with disabilities they become caught up with handling all of the items that need their attention in the present moment. We are often unable to do all of the things everyday that need to be done…work, homework, child care, medical care, physical therapy, specialist appointments all consume our time. On top of this balancing act, we try to be parents instead of case managers, social workers and care providers. Before we know it, we are in the final years of school. Where did the time go? What’s next? How do I help my teenager move on and move out? Who will his physician be when he can no longer access medical care through his pediatrician? What support will he need to sustain an independent lifestyle? HealthyTransitionsNY.org is a place that has resources for youth and parents. The Healthy Skills Checklist outlines many of the topics that need to be considered as a young person moves into adulthood. The Learn, See, Do approach offers opportunties for youth to build on their health care self-advocacy skills. This website is a valuable tool and resource for understanding terms and developing health care advocacy skills.
